Who Is Eligible
Associates working a regular schedule of 30 hours or more per week are eligible for our full-time benefit program. Associates must be actively at work on hire date and properly enroll through the Dayforce system for benefits to become effective on date of hire for medical, flexible spending accounts, dental, vision, and life insurance (basic life/AD&D and supplemental life.) Associates are automatically enrolled in LifeAdvisor/EAP services effective on date of hire. Short Term Disability (STD) and Long Term Disability (LTD) insurance is effective first of the month following 30 days from date of hire. Coverage for elected voluntary benefits (critical illness, accident, hospital indemnity) is effective first of the month following 30 days from hire date.
When Coverage Begins
- Medical, Flexible spending accounts, Dental, Vision, and Life insurance: Date of hire
- Short Term Disability (STD) and Long Term Disability (LTD), voluntary benefits: First of the month following 30 days from date of hire
Eligible Dependents
- Your legal spouse.
- Your dependent children by birth, marriage, legal adoption or legal guardianship, until the end of the calendar year in which they turn age 26.
- Your child over age 26 who was totally and permanently disabled prior to age 19 due to either a physical or mental disability. They must be incapable of self-sustaining employment, unmarried and dependent on you for more than half of his or her support, and reported as a dependent on your most recent federal income tax return. We must be notified of the condition before the end of the year in which the dependent turns 26. The disability must be certified by a physician.
Qualifying Life Events
- Birth, placement for adoption, or adoption of a child, or being subject to a Qualified Medical Child Support Order which orders you to provide medical coverage for a child.
- Marriage, legal separation, or divorce.
- Death of a dependent.
- A change in employment status that affects eligibility under the plan.
- A change in election that is on account of, and corresponds with, a change made under another employer plan.
- A dependent satisfying, or ceasing to satisfy, eligibility requirements under the plan.
- Electing coverage under your state’s Marketplace (also known as the “Exchange”) during annual enrollment or as a result of a special enrollment.
